The following forms can be downloaded then printed and completed with student details. You can submit them by email to info@dancemagicacademy.com.au, or simply bring them with you on the day that you commence classes.

 

 

Registration will be finalised and the student's position in class secure once payment of the Annual Registration Fee has been received.

Due to child protection and privacy laws, students must have a completed, signed Media Release Form on file at Dance Magic Academy. At certain times during the year, special events and some classes will be filmed or photographed with parent and guardian permission. A parent or student may set conditions on the use of Photographic or Video footage if they prefer (eg Only for Internal Use, not for Public Advertising Purposes).
